Concierge Hub for Value-Based Care

The affordable way forward to bridging data, engagement, and prevention in one platform

HealthJay's Concierge Hub transforms Life Plan Communities and Accountable Care Organizations from reactive service to a proactive, outcome-based model—the essence of value-based care.

It automates early detection, improves quality scores, reduces avoidable costs, and strengthens relationships with both families and payers—all while fitting seamlessly into capitation and shared-savings models.

What's inside the Hub?

HealthJay's Concierge Hub is a Zero-Navigation App inside a tablet that is to be wall-mounted in a hotspot at home by us. We take care of the installation, and will give simple onboarding to the residents to spare you from tech support.

Our program transforms every resident’s room into a private communication hub—a bridge between Life Plan communities and the outside world.Through an in-room tablet, residents can join automated morning wellness surveys, enjoy hands-free virtual doctor visits, connect with family and social classes, and even share feedback on dining—all without staff assistance. Audio reminders are on throughout the day to support medication and appointment adherence, and to reduce staff workload.

For staff, HealthJay simplifies operations: notes and surveys are logged digitally, family updates are sent instantly through mass messaging, and communication becomes transparent and effortless. Data is collected in each touchpoint, ready for reporting and billing. It’s not just technology—it’s a smarter, more connected way to live, communicate, and age in place.

How does the Hub lower costs and generate revenue

1. How to Generate Billing in a Value-Based Model

A. Data-Driven Billing Foundation

HealthJay already collects daily wellness, social engagement, and monitoring data — this is your foundation for billing.

•Documented Outcomes: Each automated wellness survey, alert, and follow-up note becomes a billable data point under preventive service categories.B. Billing Options

•Event-Based Add-Ons: Optional services like virtual visits or CCM monitoring can generate CPT or HCPCS codes

•Shared-Savings Participation: Use HealthJay’s outcome data (e.g., fewer hospitalizations) to qualify for shared-savings or pay-for-performance programs.

2. Working with Value-Based Care Capitation

Capitation means you’re paid a fixed amount per resident, regardless of how many services are used — so efficiency and prevention become your profit drivers. HealthJay supports this by:

•Reducing avoidable hospitalizations through early alerts.

•Documenting interventions for quality reporting.

•Using predictive analytics to flag which residents need more proactive engagement.

Under capitation:

•The less reactive “sick care” you deliver, the more margin you retain.

•HealthJay’s automated surveys, reminders, and fall detection help you achieve this without staffing up.

3. How to Monitor High-Risk Residents

HealthJay's ecosystem uses Tenovi Remote Patient Monitoring devices & Mercury AI Sensors that continuously collect data and analyze:

•Daily wellness responses (energy, sleep, appetite, mood)

•Behavior shifts detected by motion/fall sensors

•Missed responses or changes in speech tone or engagementDashboard tools can then automatically:

•Flag residents trending toward higher risk.

•Rank them by urgency for staff follow-up.

•Generate alerts or case summaries for care teams or external providers.

4. How to Mitigate High Risk

Proactive Prevention through HealthJay’s System:

•Early Alerts: Daily surveys and AI pattern recognition detect declines before symptoms worsen.

•Mass Messaging: Enables group reminders (hydration, flu shots, weather safety) without staff calls.

•Telehealth & Virtual Visits: In-room, hands-free sessions prevent ER trips.

•Personalized Engagement: Social hour participation and dining surveys improve mood and nutrition—both critical to risk reduction.

•Data Loop to Families: Keeps families aware, which encourages timely interventions and builds trust.
